It takes around 2-3 hours to unlock all of the achievements on Xbox One. Refer to the items section of this guide to figure out how to get the trophy. In your second run you can still collect these hearts just for the hell of it, but you'll still always be stuck with one heart.

@Janeator I haven't touched this game since it's launch so I remember next to nothing. Hearts make up the players health in the Minit world. Finding all the residents is something you will have to do to get the flippers, which, as far as I know, are required for beating the game.
